New Patient Information

In order to have the best chance to improve your health, you may need to change some of your lifestyle habits:

  1. Depending on the test results, you may need to follow a no sugar, no wheat, and no dairy diet. You may also have to stop eating junk food and drinking soda or pop. This takes stress off of your digestive system and allows your body to heal faster. If this diet is not followed, it may slow down your body’s attempt to heal and improvement will take longer.
  2. Part of the NES® results is a recommendation of several Infoceutical remedies that will need to be taken in order to for your body to start healing. The average patient will need to take 3-4 different Infoceuticals. You will be given written instructions on how to take the Infoceuticals, but it usually takes 20-30 minutes per day to properly take them.
  3. NES® is not a quick fix for symptoms. It will take regular treatment over a period of 4-6 months for mild conditions and maybe 6-9 months or longer for severe conditions. Most people start to see improvement in their health within the first 3-4 months. Follow up visits take place on average of every 4 weeks. To only do a treatment or two, or to just “try it out”, is a waste of your time and money.
  4. The New Patient Form can now be filled out on-line. You then submit it and a copy will be sent to me. If you do not have Internet then call me at 651-287-1311 and I can mail you a copy.

This form is required by the Minnesota Department of Health. Click on the link and and you will be taken to a page with the form on it. Fill it out and submit it. I will be sent a copy of it. HIPAA Form

This is also a required form. Click on the link and and you will be taken to a page with the form on it. Fill it out and submit it. I will be sent a copy of it. I will print it out and have it at your first appointment. HIPAA is an acronym for the Health Insurance Portability and Accountability Act of 1996. HIPAA was established within the Clinton administration’s health care reform policy, and was enacted to improve productivity of the American health care system. The law encourages development of information systems based on the exchange of standard management and financial data using EDI. It also requires organizations exchanging transactions for health care to follow national implementation guidelines for EDI established for this purpose.
